Problem

An older adult with moderate to severe Alzheimer's dementia (AD) presents for follow-up. The nurse practitioner provides education to the patient and caregiver about a new prescription. They discuss glutamate, which is an excitatory neurotransmitter in the brain that is important for memory and learning. However, in AD excessive activation occurs and can lead to excitotoxicity and death of the neurons. The new medication works by antagonizing or blocking a certain type of glutamate receptors in the brain during excessive stimulation.
