The largest blackout in US history occurred on August 14, 2003. It affected eight US states and parts of eastern Canada. Millions of people were without power, some for multiple days. After a long investigation, the cause was determined and actions were set forth.

Read the report by the NERC titled "Technical Analysis of the August 14, 2003, Blackout: What Happened, Why, and What Did We Learn?"
