Problem

The nurse cares for a 15-year-old male client with sickle cell anemia in the emergency department. Admission Notes The client presented with a 3-day history of diarrhea and generalized body pain rated as an 8 on a scale of 0 to 10. The client also reports chest pain. Vital signs are temperature 102°F (38.9°C); heart rate 122 bpm; respiration rate 40 breaths/min; and blood pressure 132/84 mm Hg. The pulse oximetry reading is 95% on room air. The client's current weight is 110 lb (50 kg). The client is currently on 3 L of oxygen via nasal cannula. The client cannot provide a urine sample at this time. A complete blood count and an electrolyte panel are pending. What assessment(s) require immediate follow-up?
