Case Scenario: Stephanie is a 52 year old female you are seeing today for nutrition counseling. She made an appointment with you today because she hasn't been able to lose weight and feels "stuck."
Past Medical History: HTN, Breast Cancer 9 years ago treated with surgery, chemotherapy, and radiation, post menopausal
GI Symptoms: constipation and complaints of bloating after meals.
Vitals:
Wt: 203 lbs Ht: 5'5" BMI: 33.8 HR: 77 Temp: 98.6 BP: 121/77
Nutrition and Weight History:
1) Stephanie consistently tracks her calorie intake and eats ~1200-1300 calories per day. She also wears a FitBit and tracks her steps and exercise. She doesn't miss a day of tracking either her food or exercise. She endorses high feelings of anxiety if she goes over her "calorie allotment" for the day; or if she doesn't exercise enough to burn a certain amount of calories.
2) She reports rules about food such as won't eat after 6pm and limits the amount of bread she can have during the day.
3) Her late husband always liked her on the "thinner side" and reports weight concerns throughout her life. She has been on several diets before and always trying to "stay thin."
4) Stephanie reports having the goal to lose 25-30 lbs. She thinks she hasn't been able to lose weight because she is past menopause now. She has been following a meal plan she found online, but her weight hasn't changed.
5) Stephanie reports being unhappy and uncomfortable in her body.
Diet Recall:
1) Breakfast at ~7:30am: 2 hard boiled eggs, ½ cup oatmeal, 4 oz fresh pressed juice, 8 oz black coffee
2) Snacks sporadically between breakfast and dinner in effort to "save" calories: popcorn, lunch meat and cheese rolled up, almonds, beef jerky, clementine oranges
3) Dinner at ~7pm: has been eating a meat and vegetable recently (for example, chicken/fish and broccoli/salad/carrots), doesn't eat carbs at dinner
4) Drinks: water through the day - is very good at drinking water and usually gets 64 oz per day, has chai tea or herbal tea in the afternoon, drinks "detox" tea before bedtime
