Sarah (she/her) is a 37 year-old G4P3 who had a repeat C-section today at 0900, 36 3/7 weeks, and gave birth to a baby boy named Walker, who weighed 6# 2 ounces. You are the night shift nurse, taking over care at 7 PM. Sarah's blood type is A+, she was GBS-, Rubella Immune, Hep -. She is allergic to penicillin. Past medical history is significant for preeclampsia in her first and second pregnancies. Fundus is firm, one finger breadth below umbilicus, bleeding is moderate. Her urine output since 8 AM is 330 ml and looks like picture attached below. She received a Duramorph spinal and ketorolac (15 mg) in the OR. Vital signs and results of her pre-op CBC are below. BP 148/90 RR 24 Pulse 80 Temp (oral) 98.4F CBC: Hgb 9.8 Hct 29.4 RBC 4.60 WBC 13.9 Plt 110 Currently rating her pain at 3/10, denies nausea. She is drinking ample water and her IV is running LR at 125ml/hour. She has eaten some crackers and tolerated them well. Walker had APGARs of 7/9 and received 30-45 seconds of PPV for low tone and poor color. He is breastfeeding, but having some difficulty latching on. He latched on well once or twice today, but mostly has been spitting up a lot and also very sleepy.
