Patient info: 48 year old male Chief complain: "Right great toe pain" History of present illness: 48 year old presents to the clinic with sudden onset of right big toe pain, woke up this am with a swollen right great tie. He states that it is difficult to walk due to the pain, pain is a 8/10 and he did take Tylenol but did not help, this has never happened before, no fever, chills. Past medical history: None Past surgical history : None Family History : Allergies : NKA Medications : MVI Social History: Drinks 2-3 beers daily -ETOJ, no smoking or illicit drug use. Review of systems: Denies recent illness. New medications Physical Exam: V/S : BP 138/82, HR 82, RR:14, Temp: 98.3 Weight: 225, Height: 5 ft 10 inches Right first metatarsal proximal joint with macular erythema, + swelling and tender with palpation, ROM decreased due to pain, + warmth, no open areas, left foot WNL.
