CASE SCENARIO: HOT GARY

Gary, 22 years old from Tondo Manila, presented to the clinic because of on and off high-grade fever (maximum of 40oC) for 6 days with associated headache, 1-2 loose stools per day, abdominal pain and vomiting. Paracetamol 500mg every 4 hours for fever which gives temporary relief. He had a sister who had the same symptoms two months ago. No travel history outside Metro Manila. Not sure about his immunization history.

Physical Examination:

Gen. Survey: Conscious, coherent, febrile, weak looking, not in respiratory distress.

Weight: 56kg

Vital signs: BP = 110/80 HR = 80/min RR = 28/min T = 38.9oC

HEENT: Slightly pale palpebral conjunctivae

Chest/lung: regular heart rate and rhythm, clear breath sounds

Abdomen: Slightly distended, normoactive bowel sounds, tenderness at the right upper quadrant, liver palpable 3 finger breadths below the right subcostal margin.

GUIDE QUESTIONS:

  2. Discuss the pathophysiology/pathogenesis of the disease.
  3. Discuss the stages of the disease and the most appropriate laboratory procedures/tests to request.
  4. At this stage of the disease, what laboratory procedures are you going to request to confirm your diagnosis? If you are going to do culture, specify the clinical specimen that you are going to collect and the best culture medium for the isolation of the possible causative agent and explain why. Include the biochemical tests for the identification of the most likely causative agent.
  5. Discuss the principle of the other tests that may be requested to help in the diagnosis of the disease. Discuss the significant results.
