Mr. Michaels is a 75-year-old man with emphysema, asthma, high blood pressure, and almost complete hearing loss. A lifelong smoker, he quit smoking a few years ago when he was diagnosed with emphysema. For some time, he has been on oxygen therapy at night. He takes several medications daily, including 10 mg of prednisone, a corticosteroid inhaler, albuterol by nebulizer, and high-blood-pressure medication with a diuretic.
Two days ago, Mr. Michaels felt sick and was running a temperature. Mrs. Michaels, his wife, took him to his primary care provider's office where he was diagnosed with an upper respiratory infection and sent home with an oral antibiotic. Mr. Michaels woke up in the middle of the night in respiratory distress. Mrs. Michaels called 911, and Bobby was transported by ambulance to the emergency department, where he is diagnosed with pneumonia and acute respiratory distress syndrome. Bobby is placed on mechanical ventilation and admitted to the critical care unit.
