Assignment:

You are a Senior Manager at Costco's head office and one of your employees who works at a retail store in Vaughan, Ontario named Janiya was injured on the job. Janiya informs you that she fell on the way into the store because no salt had been laid earlier that morning to melt the ice in the parking lot. When speaking to Janiya's supervisor, you discover that Janiya was late for a meeting and was running through the parking lot. The supervisor also informed you that Janiya entered through a side door, which is restricted to employees only, and that the customer entrance was indeed salted.
