Case Today has been an especially busy day on your unit. The unit works well under these conditions, and everyone pitches in to help. You answer a call light on a 76-year-old client who has diabetes and a seizure disorder who points to an IV site and says, "My arm really hurts here." Upon inspection you notice that the area is red, hot, swollen, and tender to touch. The area surrounding the insertion site extends approximately 6.5 cm in circumference. You check the solution for additives and discover that the solution contains 100 mg of phenytoin. You report your findings to the nurse assigned to this patient. Three hours later you ask her if her patient's arm is any better. She responds, "I'll get to it when I get to it. Can't you see how busy we are today?"
