What are the ICD-10-CM/ICD 10 PCS, Admit diagnosis and principal diagnosis and any secondary diagnoses?

This is a 65-year-old woman with a known history of COPD, on home O2 24x7. She also has a previous history of hypertensive cardiovascular disease.

She states that she has been having more difficulty in breathing, not related to activity, but exacerbated by just her normal routine. She also complains of lower extremity edema. The patient is also on hemodialysis for ESRD.

Echocardiogram done shows the patient with a 15% ejection fracture and congestive heart failure.

It was determined that the patient was in acute diastolic heart failure. She was treated with IV Lasix and improved. She underwent her normal scheduled 5 hour intermittent hemodialysis while in the hospital. She was discharged home with Home Health Nursing care.
