John Alexander is an 89-year-old male who is a 10-year resident of a long-term care center. He has not had a bowel movement for 4 days. He states, "I usually have a BM every other day." Mr. Alexander had a history of bowel surgery in 2005 for a cancerous tumor of the transverse colon. He has a distended abdomen, and hypoactive bowel sounds in all 4 quadrants. He reports he has abdominal pain and is oozing liquid stools. Mr. Alexander has a poor appetite and needs encouragement to drink fluids. He has been very sedentary lately and has not walked around the center like he usually does. The physician orders a high fiber diet, fluid intake of 2500 mL daily, oral laxative, and an oil retention enema for Mr. Alexander.
