Question: My patient has a history of (uncontrolled) diabetes type 2, hypertension, end stage renal failure, coronary artery disease and left-sided heart failure. He currently has a left ventricular assistive device (Impella) and is on continuous renal replacement therapy (CRRT). He is not a candidate for a kidney or heart transplant. He options are hospice or to get an outpatient LVAD which comes with many risks with dialysis tri-weekly. What are my priorities for nursing care for this patient/shift?
