Subjective Data: Complains of swelling in her feet and hands; Has gained 10 lb (4.5 kg) in the past 2 weeks; Complains of dyspnea and weakness when walking. Objective Data: Laboratory Data-Creatinine clearance: 8.2 ml/min, Serum creatinine: 12.8 mg/dl (1132 mmol/L), BUN: 125 mg/dl (45 mmol/L), Potassium: 6 mEq/L (6 mmol/L), Hematocrit: 20%. Chest X-ray: Pulmonary edema
