Case Study: Ming Ling, a 46-year-old Asian school teacher, has been treated for type 2 diabetes mellitus since the age of 25. She has been observed by her nephrologist for the past several years for manifestations of progressive chronic kidney disease. Eight weeks ago she had an arteriovenous fistula created in preparation for starting hemodialysis. Over the past week she has experienced anorexia, nausea, vomiting, problems with concentration, and pruritis. Subjective Data: Complains of swelling in her feet and hands; Has gained 10 lb (4.5 kg) in the past 2 weeks; Complains of dyspnea and weakness when walking. Objective Data: Laboratory Data-Creatinine clearance: 8.2 ml/min, Serum creatinine: 12.8 mg/dl (1132 mmol/L), BUN: 125 mg/dl (45 mmol/L), Potassium: 6 mEq/L (6 mmol/L), Hematocrit: 20%. Chest X-ray: Pulmonary edema
