Question: I have a sentinel even that occurred, mother goes into emergency c-section under general anesthetic, the baby is taken out, the mother's heart has stopped, and she has bled out. The mother's platelet levels were extremely low which prevented any clotting which resulted in her passing. Healthcare providers were too slow to recognize the warning signs and failed to review the mother's medical records that showed her several telehealth appointments due to COVID regarding her continued platelet level falling throughout the pregnancy.

I need assistance in identifying the needs of the patient's family after the occurrence (mother passing away after childbirth), what are the cultural variables or social impacts on patient care, and what, if any, are the financial implications for the patient's family?
