Problem

You enter Mr. Garcia's room to perform an assessment. He is resting in his inpatient bed. He is alert and verifies his name, birthdate, and that he has no known allergies. You pull back the top sheet to perform an assessment. You see the colostomy pouch has leaked because there is stool on the top sheet and his gown and a strong odor of stool. Mr. Garcia is visibly upset at the sight and states, "This thing is leaking! I can't even look at this crap running out of the bag! It stinks!" You close the door to the room and don gloves. Inspecting the colostomy, you see the clip securing the bottom of the colostomy pouch was partly open and the pouch is now half full of liquid brown stool. You remove the colostomy pouch from the appliance. Inspecting the stoma, you note that it is rosy, pink, with the wafer intact to the skin. There is no leaking from the wafer. The second stoma has mucus at the stoma site and is covered with a dry gauze bandage. "I don't know how to take care of this thing! What am I going to do when my girlfriend goes back to work?" Mr. Garcia looks away and closes his eyes. You remove the soiled sheet and gown. Proceeding with your assessment, you note active bowel sounds in all four quadrants. You ask Mr. Garcia to rate his pain on a scale of 0-10 and he states, "It's a 1/10. I am feeling ok now, I just hate this bag. I am too young for this." care plan.
