narrative introduction. You may consider discussion on your: family, hobbies, environment, and/or anything that will help the reader understand what makes you, you! This will be submitted in Role Synthesis Adaption Framework.
Biography= Twenty (20) Points
I. Traits: 10 things I like about myself - Best Traits with description of how you identified them.
2. Description of self or Self Analysis (one (1) to two (2) paragraphs)
3. Life Goals: 5 year plan (includes growth professionally, academically, and personally). Professional Experience = Twenty (20) Points.
I) Cover letter/Resume
• Listing all jobs and volunteer work in related field of study.
2) Letters of Recommendation
? Recommendation addressing student character, work ethics, ect (can be provided by instructor, clinical staff, or employer).
3) Certificates/License (Copy of Current and/or Certificates received during program)
? CPR license, CNA, or MA certificates
4) Awards (Any academic awards received during the duration of the program)
Professional Development (PD) Learning Experiences = Twenty (20) Points.
A minimum of four (4) with grades > C that Demonstrate knowledge of Basic Writing Skills
