Question

VH is initiated on warfarin (Coumadin) therapy 5mg daily. She returns to the clinic 5 days later. Her INR is 2.4 and low molecular weight heparin is discontinued. Why does the warfarin (Coumadin) take so long to reach a therapeutic INR? The existing clotting factors that warfarin (Coumadin) inhibits synthesis of have long half-lives in the plasma The patient may not be taking warfarin (Coumadin) as directed The patient may be eating foods high in potassium (K+) which will delay the effect of warfarin (Coumadin) Warfarin (Coumadin) is not well absorbed from the GI tract.
