Question 

Jack, an 8-year-old previously healthy boy presents to your office with poor appetite, nausea, vomiting and intermittent abdominal pain. He has had a poor appetite for about 24 hours, with intermittent nausea and 5 episodes of non-bilious emesis, in the last 12 hours. Jack developed right lower quadrant pain about 6 hours ago. The pain is described as sharp, moderate and worsened by activity; running and playing basketball. He denied any trauma, fever, diarrhea, cough, constipation, sore throat, dysuria or hematuria. Jack has not had any recent travels, changes to his diet. Per his mom's report, no one else in the family has similar complaints and no notes from school have been sent about these types of symptoms. Jack was born at 35 weeks gestation, with a small atrial septa defect, which closed spontaneously approximately at 12-months of age. He has been healthy and is not on any medications. He does not have any known medication allergies. Jack's vital signs included a temperature of 37.6 C, heart rate of 120 beats/minute, a respiratory rate of 20 breaths/minute. He is at 100% oxygen saturation by POX. His BP is 101/65. Weight = 38 kg
