You are a physician and your patient comes to the office for follow up after a visit to the ER with chest pain that was diagnosed as GERD. He is 48 years old, weighs 260 lbs, height 5 feet 7 inches. He has a past medical history of hypertension and diabetes. He smokes 1/2 packs/day. His family history is notable for the fact that his father died suddenly at age 59.

He is here for a follow up visit advised by the ER doctor. What treatment(s) do you provide (if any), what tests do you do (if any), what patient education do you provide (if any)?
