Sw explained to POA that the purpose of the call was to provide support listening and find out how we can help. POA shared memories and stories from pt health jounrey and from his childhood. Sw provided support and companionship during the call. Sw encourgaed PCg to share his concerns in a plan of care meeting with hospice PCM, Skilled Nursing facility Nurse Pratictioner ( Kristin), Social worker ( Laura). Pcg was able to share his memories and plan of care for mom. Clinical Team was able to share hospice and care goals for mom. POA agree with goal of keeping patient comfortable and verbal consented to contuine hospice care.
