History of present problem: a lert, and oriented 49 year old male presents with epigastric abdominal pain and unrelieved worsening of nausea and vomiting over the past two days. rate abdominal pain as a dull, continuous ache at a pain level of 5/10. reports increasing fatigue over the past several months and decreasing appetite. has had very little to eat or drink over the past 24hrs. past medical history: 2 year history of hypertensive controlled with hctz, hepatitis B 6 years ago. social history: divorved, living a lone, unemployed for 6 months, smokes cigars 1-2 daily. priot present illness, drank 3-4 glasses of vodka with orange juice nightly. physical assessment: skin warm/dry, lips dry, thick, stickly saliva, icteric sclera, PERRLA, spider telangiectasias noted to right cheek, breath sounds clear, heart sounds regular, rapid. pulses strong, equaly, 1+ edema noted to mil calf, abdomen distended with fluid wave
