Question

Mr. CD, a 68-year-old male, underwent coronary artery bypass grafting (CABG) x1. Mr. CD was extubated 8 hours after surgery, has remained neurologically intact and his pain is well controlled. On post-operative day 2 Mr. CD required 2 unit of packed red blood cells for a hemoglobin and 6.8 and his heart rhythm converted to atrial fibrillation with a rapid ventricular rate (a-fib w/ RVR), his blood pressure has remined 110/80.

1. What is preferred method of treatment and what pharmacologic agents could be used to achieve this.

2. It is now post-op day 4 and Mr. CD has remained in rate controlled atrial fibrillation for > 48 hours. He has not required any further blood transfusions and his hemoglobin has remained above goal. The advanced practice provider now needs to initiate anticoagulation therapy for Mr. CD. Discuss oral anti-coagulation options for Mr. CD and the positive and negatives of these options.
