Aim of assessmentThis assessment item aims to develop:•a critical understanding of the processes involved in the design, implementation and evaluation ofa primary health care oriented health program;•increased understanding of primary health care theory and its application in practice;•skills in research and analysis of literature; and•effective written communication skillsDetailsYou will be required to identify and analyse a contemporary health issue that requires attention and action at a local or national level. The health issue should reflect a primary health care orientation. You will then be required to develop a program that will bring about positive change in the area. Indeveloping this program, please ensure that you will discuss the various stages of planning,implementation and evaluation of the program, and how these stages have been informed by relevanttheories.Please note: All work must be appropriately referenced.find the health issues and describe why it is significant in that area or communityanalyse the issues in relation with social determinants of health , primary health care and its principledevelop a program which needs to address what program it is?why you choose it and how it will help to reduce the selected health issues of that area r community?after developing and planning of the program illustrate how you will implemnet the program and evaluate the program.
