Question: Based on our consultation meeting with the MCPHS Division of Student Affairs and our newfound knowledge and understanding of the three indicators (food and financial security, loneliness and belonging, and sleep hygiene), please answer the following question with your selected indicator for the project.

You are responsible for writing a single-step survey. Ten questions are quantitative-based, and ten questions are qualitative-based (meaning they cannot be answered with a single response). In the submission text, first explain the purpose of the assessment (do not go too broad). Then, include your quantitative-based scale(s). Finally, list - in bullet points - the two groupings of questions that you designed. Be sure that you have tested these questions on someone so you can understand what adjustments may be necessary.
