Patient is now under constant observation status after physical altercation with another peer, nevertheless he was asleep at the beginning of this shift, the patient was seen sleeping in bed and continued to sleep all night, showing clear signs of life at the time of this entry. No harmful or negative behaviors were observed or reported, and safety checks were conducted every 15 minutes. The nursing team will persist in observing and offering necessary support throughout this shift.
