I need help in completing my concept map about GLUCOSE REGULATION.

I have answered questions 1-3 but I need help filling out questions number 4 to 6 regarding the patient written in number 1 (which is the patient information)

A 27-year-old pregnant woman presents to the hospital with complaints of mild headache that started today and elevated blood pressure. She reports increased thirst and frequent urination that started a few days ago. Her medical history includes non-insulin dependent type 2 diabetes mellitus.

3. Recognizing cues (signs and symptoms): List at least six critical cues that are significant and could impact the patient and condition.

4. SMART GOALS: List at least (3) SMART GOALS with appropriate interventions that will positively impact client outcomes and are appropriate to the care of the client.

5. Taking action- (What will I do?) Describe a minimum of six (6) nursing interventions appropriate for clients with Type 2 Diabetes Mellitus. Define how each interventions would be performed, requested, communicated, taught, etc.

6. Evaluating outcomes: (Did it help?) Describe how the nurse will determine if each of the nursing interventions in the 'Taking Action' section for Type 2 Diabetes Mellitus will be evaluated to determine their effectiveness. What signs would point to improving/declining/unchanged health status?
