Of all the theories, the one that seems the most all-encompassing of all types of causation would be the Multiple-Cause Theory. Plus, it is literally in the name. It lays out that just removing one element of the events leading the incident will not necessarily stop the incident from happening again. It also accounts for human error as a cause without having to victim blame, and this theory helps contribute to taking preventive measures to hinder future incidents. None of the other theories acknowledge that the causation is not always linear and could have a spiderweb of multiple causes and effects.
I have been injured by the cot auto-loader in the ambulance. We were loading a patient at the ER for a transfer and the mechanism of the auto-loader failed. The control arms stopped lifting the patient halfway up. It was happening a lot that year. While I was resetting the mechanism and manually trying to move the control arms, a coworker, in an attempt to help, walked up and pressed the down button for the arms right when I finished resetting it. The arms came down on my hand causing a minor crush injury. I had to get 8 stitches on one finger and where a splint on another. All the auto-loaders got serviced right after that... Multiple causes for that incident, but at least it lead to better machine maintenance and safety practices.
