Question

Client has multiple myeloma and is being treated with chemotherapy. Client reports the onset of fever, chills, and a cough that started 2 days ago. Cough is productive for thick yellow sputum. Client reports feeling achy "all over" and lethargic, has a loss of appetite, and experiences shortness of breath with activity. Client is alert and oriented to time, place, and person. Crackles and wheezing auscultated in the left lower lobe, clear breath sounds in the right lung, and pleural friction rub noted. S1 and S2 heart sounds heard on auscultation of heart, with no murmur noted. Skin intact, hot and dry to touch. Radial and dorsalis pedis pulses 2 bilaterally. Abdomen not distended, normoactive bowel sounds heard in all 4 quadrants. Oxygen saturation 92% on room air. Vital signs: temperature, 101.1°F (38.4°C ); heart rate, 102 beats/min and regular; respiratory rate, 26 breaths/min; blood pressure, 138/86 mm Hg.
