The nurse in the pediatric clinic is providing care for a toddler client accompanied by the parent. Visit Notes Growth Chart 12 months: Client is sitting in parent's lap, playing peek-a-boo. Smiles at nurse but cries when taken from parent to measure weight and height. Parent states baby can wave, pull self up to stand and has taken "a couple of steps." Client eats three meals a day and will snack on soft finger foods. The parent describes the baby's appetite as "really good" and states the baby's favorite foods are oatmeal with mashed bananas, scrambled eggs, applesauce, and avocado. Baby has transitioned from breast to a cup and has just started to drink cow's milk. Multiple vaccines given per vaccine schedule. Client is up to date on all vaccines. Next visit planned for 18 months. 18 months: Client walking and is playing with soft blocks in exam room. Parent states baby is walking well and "gets into everything." The client can point to several body parts and has a vocabulary of around 10 words. The parent says the favorite word seems to be "no!"
