Sarah is a 24 year old female, brought via ambulance to the emergency room and subsequently admitted to the psychiatric unit where you work. This is Sarah's first hospital admission for mental health issues. She reports that she had "some mental health" treatment with her doctor in the community.

On examination she is disheveled, her clothes and hair are messy and unkempt. She speaks softly, slowly and makes little or no eye contact. She reports she doesn't eat much and isn't hungry very much. During your interview she became increasingly agitated and tearful stating that she was a "terrible crazy person who is always angry or sad". She reports that she had a recent breakup and one of her favorite uncles just died. She tells you her family called the ambulance because she told them she didn't care if she lived or not.
