Question

At 5 minutes, the APGAR score is 9 with acrocyanosis still present. The umbilical cord is clamped and cut. You placed the baby on Mom's chest and both mother and baby are bonding. You notice that a small bright red gush of blood from the birth canal occurs and it appears the umbilical cord is lengthening. The placenta is delivered and no placental fragments are noted. Within 10 minutes, you are preparing to assess Mom's vital signs and complete a fundal assessment. Vital signs are currently: BP 135/74 mmHg, HR 85 bpm, RR 16/min, Temperature 99F, O2 saturation 98% on room air. A lactation specialist is assisting Mom with her first breast feeding and you begin to assess the fundus. It is noted that the fundus is currently boggy and displaced to the right. What does this indicate and what would the nurse's next action be?
