Mr. Wallace is a 58 yo AAM who called the office to be seen via telehealth for med management. Patient consented to telehealth. CC 'I am here for med refill". Of note patient was last seen six months ago. He reports he has been taking his meds as prescribed since he had extra and refills from his previous provider.Patient reports he has been to many appointments with his PCP and specialists for his lung issues. He reports his PET scan was negative and reports he only have a lung nodule that needs continous follow up. He reports he is experiencing sadness, low motivation, and crying bouts and lack of energy. He reports he is taking care of his elderly mother and the mother of his children has cancer and nobody told him until last month and that hit him very hard. He reports his sleep is not good.He reports his appetite as good. He still uses a cane to ambulate. Support and psycho-education provided. Pt. counseled on medication/appointments compliant, appropriate, exercise and rest. Pt. verbalizes understanding. All medications refill.
