Although you have finished all 8-weeks of data collection on your core interventions, you have one more piece of data to collect prior to closing out the implementation phase of your project. In DNP851 (Planning), you completed a team survey. Now it is time to revisit that survey and check back in with your team. How has participation in this project changed their knowledge and attitudes? How is the team functioning now compared to 3 months ago? What is their perception of the work done over the past 8 weeks? And then, how did their engagement impact your results? So now, pull up the team survey you used in Planning/DNP851. Edit for wording to reflect post-implementation questioning. Feel free to remove 1 or 2 "old" questions that no longer apply and/or add 1 or 2 "new" questions to strengthen your survey - but remember to keep the core of the survey unchanged. It will be important for you to compare the results from DNP851 to now.
