The one ethical dilemma presented in this case study relates to the principle of client self-determination as outlined in the NASW Code of Ethics. Social work professionals are held to respect and promote the right of clients to self-determination (National Association of Social Workers [NASW], 2008, Ethical Standards 1.02). Ray adamantly refuses to consider going to a nursing home or other care center despite the challenges of caring for him at home. Jean, his wife, and primary caregiver, feels guilty about the possibility of sending Ray to a care facility due to her own experience with caregiving and her commitment to honoring Ray's wishes. As a social worker, the ethical dilemma arises in balancing Ray's autonomy and right to make decisions about his care with the duty to ensure the well-being of both Ray and Jean. In addition, as social workers, honoring Ray's autonomy means respecting his decision to remain at home despite the challenges it presents. There are concerns about the toll caregiving is taking on Jean's physical and psychological well-being, as well as the potential risks to her health.
