Client states, "The medicines you gave me a few months ago have been working so well. I do not know what could have happened. My head is killing me and I cannot stand to be in a room with lights on." 0915 Medication administered as ordered and client placed in a dark quiet room with cool compress. 1000 Client reports they are feeling better. Discussed possible triggers with client and client states, "I got a new job, and it has been very stressful lately. I also have not been able to exercise much, but I have been doing intermittent fasting to help with my weight loss." Client reports drinking three diet sodas daily, drinking two glasses of red wine in the evening, and eating a high-protein diet.
