The patient will effectively manage symptoms of the chronic condition and prevent exacerbation. Empowering the patient to take control of their health, prevent worsening of their condition, and improve their overall quality of life through self-management and personalized care. Since the patient has COPD for care plan word this nursing intervention/rational better based off this: This safety care plan addresses the risk of an injury for a 71-year-old male admitted to the hospital with a cough, sputum production, and a subsequent diagnosis of COPD and Acute Pulmonary Embolism (Confirmed by CT scan). He lives at home with his family and has a complex medical history, including chronic back pain (Spinal stenosis), hypertension, type II diabetes, obstructive sleep apnea, chronic kidney disease (CKD), gastroesophageal reflux disease (GERD), benign prostatic hyperplasia (BPH), mood disorder, and fibromyalgia. Additionally, he experiences pain, depression, and anxiety. Subjective Data: The patient is a 71-year-old male who has reported a progressively worsening cough and sputum over the past week. He lives at home with his family and advises he has a history of chronic back pain due to spinal stenosis, hypertension, type 2 diabetes, gastroesophageal reflux disease, benign prostatic hyperplasia, mood disorder, and fibromyalgia.
