Rephrase. SW spoke with PCG concerning Hospice discharge. PCg reported that the pt might be doing fine but if Skilled nursing does show up for six to seven days pt has declines. Sw explained that pt has Skilled nursing visits once a week and that the clinical team as well ads the Medical director review pt chart and medical record and there was no significant declines reported or observed and that pt has to be discharge from hospice due to medicare guidelines. SW provided education and support from home hrealth services pcg declined home health services at this time. SW explained the nomic form and asked pcg if she SW could stop by for signatre or email pcg said either one. SW also reviewed the appeal process. Sw called to drop by with NOMIC form no answer and voicemail was not setu for medssage to be left. sw will contact pcg agin in the morning.
