Read the following scenario: (again we are referring to the case scenario we used in Week 1 with Abby)
You have been meeting with Abby for a few weeks. In her most recent session with you, she shares some information with you that raises a reasonable suspicion of child abuse or neglect. After speaking with your supervisor, you determine that while Abby is not in immediate danger of abuse or neglect, it is still necessary to file a child services report.
Assessment Deliverable
Compose case notes and a separate child services report that can be added to Abby's case file. Keep your audience in mind for each document you compose.
Part 1: Case Notes
Brainstorm a scenario to explain why Abby may be at risk of child abuse or neglect. Consider these examples:
Abby's mother pushed her during an argument about attending church.
Beth's boyfriend, Diego, came home drunk and threatened the children with corporal punishment.
Abby confesses that she goes to an adult neighbor's house on the days she skips school.
Complete the SOAP Notes form. Be sure to:
Summarize the recent conversation you had with Abby.
Summarize your concerns about potential child abuse or neglect Abby may face.
As needed, incorporate key topics or issues discussed to provide a holistic perspective of the client.
Note: Social workers routinely complete case notes; however, the format will be determined by the agencies they work for. You may use the SOAP notes form provided or use a format of your choosing.
Part 2: Child Services Report
Complete the Child Services Report form. Your report should:
Explain the significance of your concerns.
Why do you believe Abby is at risk for child abuse? How are her basic needs being violated or neglected? How might Abby be affected physically, emotionally, or mentally if child services does not intervene?
Maintain an objective tone.
Avoid personal and/or subjective language.
Remember to review your case notes and child services report for correct application of grammar, punctuation, and spelling.
