Question: Students are to complete an assessment that a human services worker at an agency might complete for a new client. After reading the chapter on assessment and considering all of the important components to assess with a new client, students will view the video clip in the Watch folder and write up an assessment based on the interview.

You should gather basic information that addresses physical, mental, emotional, social, and spiritual issues if these are conveyed in the interview you watch. You should also include space for a brief clinical summary (no more than 1/4 typed page) at the end of the assessment where a human services worker might summarize the key issues to be addressed in the worker-client relationship. Be sure to make space for the case worker to sign and date the form on the bottom of the last page. Document in the third person by avoiding saying "I" etc.
