Clinical Assignment- Urosepsis:

You are informed by the charge nurse that you will be assuming care of a new patient being transferred from the Emergency Department. Pre-transfer written report includes the following information: 67- year-old male, transferred from the ED department who has developed a new onset of agitation and confusion over the past 48 hours. He has not been eating or drinking well for the past few days. He has bilateral wrist restraints applied "for safety" due to his increased agitation. He has a history of BPH (Benign Prostatic Hyperplasia), hypertension and osteoarthritis. He has a known allergy to Penicillin (anaphylaxis). A foley catheter was inserted one month ago for urinary retention. Last VS from ED: BP 99/40, HR 120, temperature 38.9, RR 24, SpO2 95% on room air.
