Question

As a WHNP, a rapid assessment for this patient with sudden onset of severe abdominal pain and vaginal bleeding, I would assess vital signs, perform an abdominal assessment to determine the area of pain for the pain and to determine if the pain radiates, pelvic exam, neurological assessment to assessment for dizziness, change in vision, and/or headache. I would also assess for new onset of SOB, lethargy, or diaphoresis. I would also question if this has happened before or if this abdominal pain and vaginal bleeding is due to the start of her menstrual cycle, and to assess the amount of vaginal bleeding based on the number of pads used within an hour, I would also determine if there was any recent vaginal or pelvic trauma or surgeries or discuss if she has any blood clotting disorders.
