The patient lives in a mental health group home. The patient currently takes 25mg of olanzapine every day and has been on this dose for about 6 months with previously good effect. Four days ago the patient had aggressive behaviors and was brought to the ED. The patient was admitted to your inpatient facility yesterday when a bed opened up and this is your first meeting with the patient. You will interview the patient and then spend at least 5-10 minutes explaining the assessment and treatment recommendations to the patient's sister, who is the guardian but does not have any medical background. If you want additional information such as lab values or testing, you may ask and patient will tell you if it's available. Chief Complaint (CC): ""I know I wasn't myself at the group home. I feel better now and am ready to go back." Exam is as follows: Vital Signs: All WNL, BMI 32 Patient medical history is pertinent for DMII and obesity LABS: CBC, CMP 5 months.
