Your facility performs a quantitative analysis of each health record and a qualitative analysis once a month. During the qualitative analysis for the prior month, it was noted that there is a problem with record completion. The facility's policy states that no more than 20% of the records should be incomplete within a 30-day period. The results of the analysis showed an incomplete chart rate of 40%. The responsibility of chart completion falls on the HIM Data Analyst in collaboration with the appointed Physician Champion and Chief Medical Officer.
Your facility has recently implemented an electronic work que that allows health records to be routed to HIM for processing. After analysis, the records are routed all healthcare professionals who have deficiencies to allow them to access, complete, and authenticate the health record from any accessible location. The health record is then routed back to HIM after completion.
The Medical Staff Bylaws state the following: Any healthcare provider that does not complete their charts within 14 days after discharge is placed on the deficiency list, which is also shared with the Physician Champion and Chief Medical Officer. The Physician Champion will reach out to the providers on the list to inform them of their charts sitting in the que and encourage them to complete their charts. If a health record remains incomplete on the 29th day of the month, the healthcare provider's privileges are suspended until all their records are complete.
