Instructions: Case 3 is what my assignment is based on and the pharmaceutical care plan should be completed using the case review template provided. The font style and size: arial 11. Pages should not exceed 11. Please Note, this is a pharmacy student piece of work. Sources such as eMC, medicines complete, renal handbook for medicines should be used and also journals should be used for the SOAPs. Lastly, i have provided an example of a previously submitted pharmaceutical care plan, please note this is based off a completely different case.
