Context: 30Y/o WF here in f/u with DUB, she bleeds with IC, she has a new partner but has used condoms 100%. Her BP is up as she takes HCTZ it remains uncontrolled today. She is concerned she has a yeast infection, c/o White vaginal d/c. She thinks she need to increase her Zoloft to 100mg. She is sleeping well since increasing her Trazodone. But her Metformin hurts her stomach once she bumped up to bid dosing. She cont. to fight the keratotic pilaris but is using the Lac-Hydrin. Otherwise, denies headache, visual change, chest pain, shortness of breath, abdominal pain, urinary symptoms, muscle or joint pain, rashes, numbness, tingling or weakness. Denies thoughts of harm to self or others

PE: pelvic performed, vaginal D/C yellow, watery and runny, no clumping or thickened cottage cheese like discharge. Cervix is friable once the Q-Tip touches the os. Neg. CMT. A1C point of care testing 6.7
