Rephrase. Sw attempted to reach PCG via phone no answer and unable to leave message because message stated phone was unavailable. Skilled nursing reported that several times he arrived to the home and pt was locked inside the home alone because while Skilled nursing was writing note for pcg, pcg was pulling up in the drive way. Sw rescheduled visit due to no answer from phone call attempts. SW will discuss with PCg the dangers of leaving bedbound pt in the home alone and also about pcg inadequately managing patient disability income. pcg has anot been paying the morgtage, water bill , and lights and has recieved several disconnection warning and seems not to comprehend or understand the severity of mismanaging funds. Sw has attempted to engage patient other daughter but she has not been receptive of returning calls or meeting with SW. Sw will meet with clinical and family services team to disccus APS referral.
