According to the article Columbia Disaster: What Happened, What NASA Learned, "An investigation board determined that a large piece of foam fell from the shuttle's external tank and breached the spacecraft wing," (Howell, 2017). I feel that this issue was in the hands of the NASA team. Even though they suspended the space shuttle flights, they still put their astronauts at risks and therefore their lives were taken away. They should have managed their projects wisely and made sure that everything was set and in placed before taking off. It is just like when a parent is about to go somewhere with their baby and before they pull off to drive, they make sure that the baby is strapped tight and secured before they pull off to go out. I believe that if NASA had done this, then the astronauts would still be here today.
