Shift to Value-Based Health Care Prior to 1983, hospitals billed patients on a cost-plus basis, charging for all supplies and resources used by an individual patient. After 1983, this shifted to billing for an average cost for patients with similar diagnoses, with hospitals receiving larger reimbursements based on quality care measures (DeNisco, 2019). This value-based shift in healthcare increased patient satisfaction and improved patient outcomes by focusing on the quality of care rather than the quantity of costs. However, hospitals must balance providing quality care with maintaining profit margins, especially in areas that serve underserved populations. As a result, Advanced Practice Nurses (APNs) have an increased responsibility to provide preventive care services, follow up with patients to ensure adherence to care plans, and decrease hospital admissions and lengths of stay to prevent unnecessary treatments and costs. To keep up with evolving trends, APNs must assume leadership positions and become involved in policy and procedure making. In the future, we will likely see more APNs in government roles and as directors in healthcare organizations. Nurse leaders can provide "disruptive innovation," defined as "improvement that originates in low-end or new-market footholds and pushes incumbents closer.
