Co-workers who are close to the person who did wrong often ignore the problem and don't check on the accusations very often. Many organizations don't have clear rules or plans for how to respond. Also, organizations create an environment where keeping their image safe from scandal is more important than helping the victims of reported abuse. Also, sexual abuse in institutions can happen with several people committing acts within the same organization, and there can be many victims. The abuse usually happens for a long time before anyone tells a person in charge who can help.
